Does the notification need to be sent to the superintendent of the county or to the school district that one resides within?

The notification is sent to the school superintendent of the school district where you live.

Where do we get the form to send in? I cannot find it anywhere on the Columbus Public School website.

Use this form at http://www.cheohome.org/

Go to the menu and select “Homeschool Laws” and then “Notification Form (PDF)”
